What Types of Policy Payment Options are Available?
The premium is to be paid in full with your application. Premium payment can be made by credit card ( M/C Visa or Amex).
Some products will have a premium funding option, additional charges will apply and your premium can be paid over 12 months.
The premium funding is a separate agreement between you and the premium funder.
What is the Premium Payment Plan?
The premium payment plan is a separate premium funding / loan agreement between you and the premium funder

The premium payment plan enables you to pay your premium over 12 months in equal installments. An administration fee of $50.00 will be added to your 1st installment. You will need to complete an application form which will be forwarded to you by email when you select the premium payment plan option. All you have to do is sign the forms where indicated and include your payment details and return to PO Box 140, Salisbury, Qld 4107 and on receipt of the completed form a Certificate of Insurance will be forwarded to you. What does Involuntary Unemployment cover?
In the event that You are made Involuntary Unemployed, subject to the cover selected by you and the terms and conditions of
the policy we will pay you a monthly benefit of either: $2,000 - $3,000 or $4,000 The monthly benefit will be payable for a
maximum period of 3 months for any one period of Involuntary Unemployment.The maximum total benefit amount over the Period
of Insurance is limited to 6 monthly payments.
What does Accidental Injury cover?
Provides a cash benefit in the event of an accidental injury which solely, directly and independently of any other condition
results in any of the Events listed in the Table of Benefits as listed in the PDS and occurs with 12 months of the Injury.
We will pay You the compensation specified for that relevant event based upon as a percentage of the Sum Insured by You as
evidenced in your Policy Schedule.
What does Trauma cover?
Subject to the selection of Trauma cover by you If you suffer one of the Trauma Conditions listed below we will pay you the
amount listed as evidenced in your Policy Schedule for that condition, subject to the policy terms and conditions.

To apply for cover you will need to read the Product Disclosure Statement (PDS) and the policy Terms and Conditions
The Application Form will need to be completed. If we accept your application, a Policy Schedule will be sent to you.
What is the Eligibility Criteria?
Eligibility for this insurance is subject to the following conditions:
Is there a Waiting Period(s) before I can make a claim?
How are the Premium Rates Calculated?
The premium rates are calculated based on your selection of product, coverage limit and policy term.
Can I have a Joint Policy that covers my partner and myself?
Joint policies are not available. Each person requires their own individual policy.
Is a Medical Examination required?
No Medical Examination is required however no cover is provided for any Pre Existing condition.
What is a Pre Existing Condition
A Pre existing Condition means any sickness, disease or disability or other condition relating to your health of which
you were aware of during the 12 months prior commencing the REBOUND policy. Please read the PDS for further detailed
information.
Are there any Claims Limits?
Claim limits apply and these are based upon your product selection. Please refer to Product Disclosure Statement (PDS)
for further information and product options. Your selected product options and limits will be detailed in your policy
documentation.
Can I Cancel the Policy?
You can cancel the policy at any time by informing the Insurer in writing. A cancellation fee of 10% of any refundable
premium applies to all cancellations.
Can I Cancel and get a Full Refund?
Providing you inform National Underwriting Agencies in writing to PO B0X 140, SALISBURY QLD 4107, within 14 days of
inception of this policy we will refund your payment in full.

All claims have to be submitted in writing. All you need to do is call 1300 138 560 and we will inform you of the process
What do I do if I have a Complaint?
Any enquiry or complaint relating to this insurance or services should in the first instance be referred to National
Underwriting Agencies Pty Ltd (NUA) on 1300 138 560. NUA's Complaints Manager who will handle your complaints or disputes
about the insurance or services. Should the NUA Complaints Manager be unable to satisfactorily resolve your complaint or
dispute they will advise you of the various external bodies available to assist you.
